An Academic Integrity Alert form can be obtained online via Faculty One Stop. Click here to link the Faculty One Stop Guide regarding plagiarism and academic integrity issues. The guide includes a digital copy of the Academic Integrity Alert form. Paper forms are also available in the administration office.

These forms are completed when a student has plagiarized and the instructor intends to report it.
